| St. Anne’s Salvatorian Campus is a
fully licensed long term care facility in the State of Wisconsin
offering both skilled and residential care services. We are
dually certified as a Medicare and Medicaid facility.
The continuum of care that can
be found here under one roof is a decided benefit should a
resident require differing levels of care. For example, an
assisted living apartment dweller might need care that can
only be provided with temporary or permanent placement in
the nursing home. At St. Anne's that is only a trip down the
hall. Or, if an individual comes to St. Anne's for rehabilitation
but will not reach the level of independence needed to return
home, our assisted living apartments may be the answer for
those needs.

Skilled Care Services
Currently 115 beds are available within St. Anne’s
Home for the Elderly, our normally deficiency
free nursing home which is dually certified Medicare
and Medicaid. Nursing home rooms are housed on
the south end of the campus close to therapies;
the dining room which serves physician ordered,
dietitian and speech therapy approved therapeutic
diets; activities areas, medical, dental and podiatry
and social services. We accommodate short and

Assisted Living Apartments
St. Anne's assisted living apartments
are located on the north end of the campus.
In March 2005 we opened our second section of
assisted living apartments bringing the total
number of apartments to 39.There are two sections
of apartments: Each section has its own dining
room, lounge space and staff.
- Tivoli Terrace – 24 one bedroom apartments
– traditional & open concept
- Jordan Hall – 15 units – four
floor plans - studio, deluxe studio, one bedroom
and two bedroom apartments.
St. Anne's assisted living is “small enough
to… really feel like family.” Residents
tell us “I feel safe and people here care
about me.” “I have privacy when I
need it and community when I want it.” Our
staff provides highly personalized care in a ’home’
large enough to offer everything a resident might

 Rehabilitation
& Pharmacy Services
Physical therapy, occupational therapy and speech therapy
services are provided on site at St. Anne’s by
professional rehabilitation (therapy) staff as required
by Medicare Part A, Part B and Medicaid.
Pharmacy services are also provided by Covenant Health
Care Pharmacy Long Term Care Services.

Dietary Services
Our dietary department receives considerable praise
for the quality of food they serve. Therapeutic diets
are made available when ordered by the physician and
planned by our registered dietician. We attempt to incorporate
resident ‘likes’ and ‘dislikes’
into diet planning.
Family members and guests are welcome to eat with residents
providing they have made prior arrangements. Meal tickets
can be purchased at the reception desk.
Catering services are available for special events. |
